SUMMER ENROLLMENT FORM

If you would like to enroll multiple kids, please complete an enrollment form per child.

New Students- Class fees are due at the time of enrollment to hold your child's spot in class. 

Returning Students- Class fees for the 1st month are due by June 1st.

Shrek Jr flat fee and show tshirt fee are due before auditions, which will be on May 15th and 17th.

*Shrek Jr Requirement- Students must be enrolled in at least one other singing, dancing, or acting technique class at DCT.
